Important Notice to Patients
As of July 1 2026, all patients will be required to come to reception immediately after their appointment to sign an Assignment of Benefit form.
This form provides your consent for Medicare to pay the bulk-billing benefit directly to the clinic for bulk-billed appointments.
This process will only take a moment and is required by Medicare for all bulk-billed appointments.
